405-C:10 Reporting Requirements. –
Each joint self-insurance arrangement shall file a report verified by the oath of an officer or trustee, annually within 4 months of the end of the fiscal year or within such extension of time as the commissioner for good cause may grant, with the commissioner providing:
I. Details of any changes in the articles of incorporation, bylaws, charter, trust agreement, or other agreement among the participating entities;
II. List of all current officers or trustees;
III. A description of the program structure, including participants' retention, excess insurance or reinsurance limits and attachment points;
IV. An actuarial analysis, performed by a member of the American Academy of Actuaries qualified in the coverage area being evaluated, assessing the adequacy of plan funds and the reserves necessary to be maintained to meet expenses of all incurred and incurred but not reported claims and other projected needs of the arrangement;
V. A list of contractors and service providers;
VI. The financial and loss experience of the arrangement; and
VII. Such additional information as the commissioner may reasonably require to determine compliance with this chapter.
