It is extremely important to review every maternal mortality to understand if it was preventable. We must learn what the health care system did well, what lapses occurred, and what happened beyond the control of health care providers and the patient’s own personal responsibility. Although these are always very tragic situations, by using a kind and supportive review, the team will obtain important information that will ultimately improve care.
